Mentee Application Instructions

Dear Joanie Girl’s Heart Candidate and Parent/Guardian,

Thank you for your interest in our Heart Matters Program. This is a wonderful and exciting time in your life; having a mentor is rewarding and a great opportunity to help you reach your full potential. The screening process is very important and is an integral part of the program requirements. Please read each document completely and answer all questions.


  • Minority Female High School Sophomore
  • At least 3.0 GPA
  • Can commit to program for 3 school years
  • Interested in pursuing a healthcare career- Medicine, Dentistry, Nursing, Pharmacy, Chiropractic, Mental Health, Healthcare Administration
  • Must be available two Saturdays per month
  • 3 letters of recommendation
    • Guidance Counselor
    • Teacher
    • Personal (non-parent)
  • 500 word essay introducing yourself and detailing your interest in pursuing a healthcare career
  • May be eligible for a 5 day mission trip during spring break of Junior year
  • May be eligible for international mission trip Senior year

Application Process:

Step 1: Gather support documents:

  • 500 word essay describing yourself and your interest in pursuing a healthcare career
  • High School Transcript
  • 3 letters of recommendation-
    • Guidance Counselor
    • Teacher
    • Personal (non-parent)
  • Contact and Information Release Form

Step 2: Complete the online application:  **Please be sure to have all of your support documents prior to starting the application. You will not be able to save and return to your application.**

Step 3: Verify all documents are attached to your application. Incomplete applications will not be processed.


Application and materials are due by November 12, 2014.

Please allow one week for application processing. Final candidates will be contacted via phone and/or email by November 14, 2014 by 5:00 p.m. All candidates and Parent(s) must be available for interviews on November 15, 2014.
